The basic personal details of the nominee and nominator (if you’re not self-nominating).
The recent employment history and qualifications of the nominee.
Responses to the selection criteria (max. 300 words each).
Contact details for two references.

If shortlisted, two (2) formal references are required to support your application. A formal reference is an endorsement outlining how the you or the person you nominate meets the criteria. A letter of endorsement cannot be written by the nominee or nominator. Both letters of endorsement must be written by someone who has a professional affiliation with the nominee eg. Current or past manager or coworker.
